The medical clinic at COFMC offers several types of care for our new and established patients such as primary care, women’s health, Take Charge program, DOT physicals, school/sports physicals, pediatrics and immunizations. We also have same day appointments available. We file all insurances including Medicare and Medicaid. We also have an income based program for un-insured patients.
To apply for the income based program, one of the following proof of income must be presented at the time of check-in:
- Current paycheck stub
- Social Security Statement
- Letter from DHS with the financial services you receive
If no proof of income is available, we will accept a notarized letter from the person/place that is assisting you with your financial care. New patients need to arrive 30 minutes prior to the scheduled appointment time. Established patients need to arrive 15-20 minutes prior to the scheduled appointment time.

COFMC Behavioral Health Department provides outpatient mental/behavioral health services for children, adults, and/or families throughout the immediate community and the surrounding areas. Regardless of the diagnosis, the Behavioral Health department is dedicated to providing high quality services in a safe and supportive environment where all consumers are treated with dignity and respect.
Services available include assessment, evaluation, prevention and education, individual and family therapy, medication referral and support, psychosocial rehabilitation, day treatment, case management, risk assessment for pain management services, consultation and referral, and crisis intervention. All services provided are administered by qualified professionals and are client specific and goal oriented based on the identified need.

Dental
DENTAL - HOURS

COFMC dental offers the following services: fillings, extractions, crowns, bridges, full dentures, partial dentures, simple cleanings (adult and children), deep cleanings for adults (scaling and root planing), fluoride treatment, root canals, invisalign, and comprehensive orthodontics.

Our pharmacy is pleased to offer you fast friendly service, pharmacy consultation, computerized tax and insurance records, medication screening, a convenient drive thru window, patient assistance programs, and special pricing for patients utilizing COFMC providers.
We accept most insurances; however, we cannot fill Medicare Part B prescriptions.

Ancillary services are provided on site and are available for current COFMC patients. Sliding fee, Medicare, Medicaid, and most major medical insurances are accepted for ancillary services.
